Note:
The objective of the proposed project is to create a culture of sustainable change by assisting people to prevent diabetes and/or manage their diabetes, by providing them will instruction and tools, to mitigate the costly burden of chronic disease. The goal is to train 30 CHWs to deliver a five-week intervention "On the Road to Living Well with Diabetes (OTR)" and pilot test it with 300 participants (10 people/group) for future implementation/utilization within southern Doña Ana County, New Mexico (NM), west, northeast, and central El Paso County.
